The role allows you to focus on comprehensive patient assessments without the added burden of creating treatment plans or prescribing medications.

Schedule:

• Flexible shifts available any day Monday through Sunday

• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct annual home health risk assessments for Medicare Advantage plan participants throughout Montrose County, which includes communities such as Montrose and Naturita
• Assess prior and current medical history, chronic conditions, and medication regimens during home visits (approximately 45 minutes per evaluation)
• Deliver patient education regarding medical conditions and prescribed medications
• Identify gaps in care and recommend actionable solutions to address them
• Perform a variety of visit types, including annual assessments and post-discharge evaluations

Qualifications:

• Active Nurse Practitioner license in the state of Colorado
• Board Certification through AANP or ANCC
• Valid CPR (BLS) certification
• MSN degree from an accredited nursing program
• At least 6 months of recent experience as a Nurse Practitioner (strongly preferred)
• Commitment to delivering patient-centered, high-quality assessments

This role offers compensation of $80-$90 per evaluation, with additional mileage and administrative stipends, acknowledging your time in both patient care and travel.
