Physical Therapist - Home Health

Schedule

Day shift, 5x8 hours (08:00 - 17:00)

Assignment Details

Minimum of 6 home visits per day, with potential for additional visits at management's discretion. 1-2 weekends required per contract.

Requirements

• Minimum of 1 year of home health experience required.
• State licensure and BLS certification are mandatory.
• Must have valid driver's license and personal vehicle for home visits.
• COVID vaccination or approved exemption required; flu shot or declination also required.
• Experience with OASIS documentation preferred.
• Skills in assessment, cardiopulmonary/circulatory care, documentation, and various medical equipment are essential.
• Must be familiar with patient education and safety protocols.

Responsibilities

• Evaluate and document patient's condition and progress.
• Develop and implement individualized home exercise programs.
• Educate patients and families regarding treatment plans and safety measures.
• Coordinate care with other healthcare professionals as needed.
• Monitor and document patient progress throughout care.
