We are seeking a dedicated and experienced RN Case Manager to support one of our top healthcare clients in Tallahassee, FL.This is a full-time opportunity offering competitive pay, stackable shift differentials, and a generous $10,000 sign-on bonus.As a Registered Nurse Case Manager, you will be at the heart of the interdisciplinary care team ensuring patients receive optimal, efficient, and high-quality care from admission through discharge. This role blends clinical judgment, advocacy, and coordination to improve patient outcomes, support utilization review, and drive seamless transitions of care.Key responsibilities include:Conduct comprehensive assessments of patients medical and psychosocial needsDevelop, implement, and monitor individualized care management plansCollaborate with the care team to ensure appropriate level of care, resource utilization, and discharge planningFacilitate interdisciplinary team meetings to align care strategiesEvaluate medical necessity and level of care using evidence-based criteriaAct as liaison between patients, families, providers, and payersIdentify potential barriers to care and proactively work to resolve themMaintain compliance with all regulatory, quality, and documentation standardsProvide education and support to patients and families, especially for those with chronic or complex conditionsCoordinate referrals to community resources, disease management programs, and support servicesTrack trends, analyze data, and make process improvement recommendationsQualifications include:Active RN license in Florida (or compact license with FL endorsement within 90 days)Associate Degree in Nursing required; BSN preferredMinimum of 3 years of recent Case Management experience in an acute care setting OR 3 years of experience in Med/Surg, Telemetry, Neuro, ICU, PCU, or ED.
