Registered Nurse - Case Management - Hospital

Job Description

Job Summary:

Seeking experienced inpatient Registered Nurse Case Managers with acute care and emergency department background. Must be able to work cross-functionally across units to coordinate patient discharges and ensure compliance with insurance and regulatory requirements.

Responsibilities:

• Coordinate patient discharge planning from hospital to community settings, ensuring smooth transitions of care.
• Collaborate with clinical teams to assess patient needs, develop care plans, and facilitate resource allocation.
• Review patient records for insurance and regulatory compliance, particularly related to complex discharges in California.
• Function independently within various hospital units, providing guidance on discharge and care management.
• Maintain accurate documentation of clinical assessments, discharge summaries, and communication with payers and providers.

Qualifications:

• Hold an active CA RN license, verified via Nursys within the last 2 weeks.
• Possess a valid Healthcare Provider BLS certification.
• Minimum of 5 years' inpatient case management experience in hospital settings.
• Demonstrate strong knowledge of California insurance policies and county discharge regulations.
• Be able to work variable shifts including days and evenings, with every other weekend requirement.
• Have excellent communication, organization, and independent problem-solving skills.
