The Substance Use Disorder Professional/Case Manager (SUDP) will provide culturally relevant Substance Use Disorder (SUD) services to Tribal members and other clients. This position includes conducting assessments, creating treatment plans, offering counseling, and ensuring continuity of care through case management and follow-up.
Essential Duties:
- Provide substance use disorder services to Tribal members and other clients, including intakes, assessments, referrals, individual treatment plans, UAs, group and individual counselling and education and documentation of client care provided.
- Serve as the lead with Tribal member requests for case management. Including but not limited to:
- Requests for inpatient and/or referred care including emergency and/or crisis situations according to the policy and process in place to ensure the requests are processed quickly and the appropriate level of care is coordinated with external agencies and/or facilities.
- Monitor care received from external agencies and/or facilities to ensure inpatient and/or residential care received is the appropriate level of care. If the Tribal member is returning to ambulatory care provided by Snoqualmie Behavioral Health Services (SBHS) or another agency, provide the client with transition planning.
- Plan and implement follow-up or aftercare programs for Tribal members discharged from external agencies or facilities providing inpatient or residential care services, so continuity of care is in place.
- Provide Tribal members with information about addiction issues and services/programs available, making appropriate referrals when necessary.
- Attend relevant training sessions including evidence-based and promising practices for SUD and the provision of culturally relevant care to increase knowledge and skills.
- Prepare for and participate in SBHS care management meetings and/or SBHS staff meetings that support Tribal member or client’s access to care and transition planning or support services.
- Consistently document all care, education and case management provided in the electronic health record.
- Update annual paperwork for clients receiving services.
Additional Duties:
- Attend clinical supervision meetings with SUDP’s and staff cases and/or conduct educational meetings including review of individual service plans, diagnosing and meeting medical necessity within client files; attend other meetings as designated by the clinical supervisor.
- Maintain continuing education requirements per WAC 246-811 and develop a SUDP Enhancement Plan with the SUD Clinical Supervisor.

- Other duties as assigned.
- Knowledgeable and aware of historical trauma and how it impacts Native American individuals today; must be culturally competent and sensitive towards the needs of this population.
- Proficiency in Diagnostic and Statistical Manual, version 5 (DSM-5) American Society of Addiction Medicine (ASAM) criteria as it relates to diagnosis.
- Proficiency in the use of ASAM Placement Criteria and the 4 Quadrants of Care.
- Excellent communication skills and ability to prioritize and work independently, including receiving and otherwise obtaining client information from all relevant sources, strong organizational skills
- Proficiency with receiving, maintaining, documenting, or transmitting confidential Tribal member and/or client records or other confidential information according to 42CFR part 2, HIPAA, and relevant Washington Administrative Codes (WACs).
- Ability to help maintain an environment that encourages teamwork, interdependence, and ethical behavior.
- Ability to take directions and follow through with recommendations of supervisor.
- Previous experience working in Tribal communities preferred.
- Strict adherence to the code of ethics that governs the substance use disorders profession.
Education & Experience:
Education Required:
- Associate’s degree in Substance Use Disorder Studies by a Washington State Department of Health approved college
Experience:
- Previous experience with Tribal communities preferred.
- Previous experience with Resource Patient Management System, Electronic Health Records preferred.
- Updating and using relevant knowledge such as Microsoft Office Suite, Adobe Acrobat, PowerPoint, or other relevant software to achieve the duties and responsibilities for the position.
Certifications/Licenses:
- Current certification as a Substance Use Disorder Professional in Washington State.
- CPR and 1st Aid certified.
- Valid Washington State Driver’s License without restrictions.
Other Requirements:
- Must be able to pass a thorough pre-employment background investigation.
- Must agree to be subject to the Tribe's Controlled Substance and Alcohol Testing Policy to include pre-employment screening.
- If a former participant in SUD treatment, must have completed treatment and maintained recovery for a minimum of five years before employment.
Work Environment:
The work environment includes an office environment in a controlled atmosphere building. Some remote or hybrid work arrangements may be possible in accordance with the Tribe’s Telecommuting Policy.

The Snoqualmie Indian Tribe follows the Tribal Hiring Preference as outlined in tribal policies, including the Tribal Employment Rights Ordinance (TERO). TERO sets forth certain hiring preferences for qualified individuals who are enrolled in a federally recognized Indian tribe, as well as, in some cases, preference for certain family members of those individuals.
Additional Information:
- Salary, compensation will be determined based on experience and starts at $76,159.67 per year.
- This position is a regular full-time position and employment is at will.
