Salient MD Recruiting is pleased to be working with our newest client - a progressive organization founded by physicians nearly 20 years ago. Their unique model focuses on value-based care for the Medicare Advantage patient population. Our client has an extensive network of over 1,200 healthcare providers, and they are expanding their footprint.\n
\nGiven the value-based emphasis, physicians will spend more time with each patient than the typical primary care setting. Physicians focus on HEDIS and quality performance; patient utilization and access; referral management and coordination; and panel growth and engagement.\n
\nA summary of physician responsibilities is provided below:  \n
 \n

\n
• Independently provide care for patients with acute and chronic illness encountered in the older adult patient
\n
• Ability to diagnose and manage chronic diseases such as diabetes, atherosclerotic disease, COPD, CHF, obesity, etc. in the senior population
\n
• Accountable for patient care and outcomes and will appropriately seek consultation from specialists when needed
\n
• Engage with hospitalists whenever a patient is in the hospital
\n

\n
\nQualified physicians will be Internal Medicine or Family Medicine physicians, have at least 3 years’ experience in outpatient primary care experience, and will be ACLS/BLS certified.  \n
\n
\nThe compensation for the selected candidate will be based on relevant experience, skills, and qualifications as they align with the role's requirements.
