Evaluates patient's response to treatments/medications.Actively participates in Quality management program.Completes accurate and thorough visit notes via Electronic Health Record (EHR) on every patient in a timely fashion.Graduate of an accredited nursing program.Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in state of practice.Master's Degree in ministry, counseling, or social work preferred.Clinical Pastoral Education (CPE) training preferred.Two (2) years or equivalent full time experience in bereavement counseling within the last three (3) years.Valid state driver's license and reliable automobile, current automobile insurance, and be willing to operate personal car necessitated by nature of job.Experience as an LPN/LVN commensurate with one of the following preferred:o One year experience in an acute care setting within the last 24 months;o Six months home care experience within the last 12 months preferred.Valid driver's license and reliable transportationProof of automobile insurance (MVR checked)Proof of current CPR certificationAble to pass criminal background check and national sex offender clearance
