This position is a variable service provider, and you will work at clinic locations based on need.Why You'll Love Working HereTeam-Based Care Model: You'll partner with a dedicated care team that supports patients' physical, emotional, and social wellness.You'll see fewer patients per day and spend more time delivering meaningful care.Supportive Culture: We foster a welcoming, inclusive environment where teamwork and growth are prioritized.Work-Life Balance: Enjoy PTO, minimal call responsibilities, and CME time.Use Your Skills to Make an ImpactYour Role & ResponsibilitiesYou will provide comprehensive care to seniors following clinical standards.You will collaborate daily with your care team to ensure coordinated, high-quality care.You will manage referrals, hospital/SNF coordination, DME, and home health services.You will participate in daily huddles.Document care accurately and efficiently with support from quality-based coders.Ensure compliance with all licensing and accreditation requirements.What We're Looking ForRequired:Master's Degree in nursing or completion of a physician assistant programBoard certificationActive, unrestricted NP or PA license in the state of practice.One year of experience in primary careExperience with electronic medical recordsPreferred QualificationsActive DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbersFamiliarity with HEDIS quality indicators and Medicare guidelines.Scheduled Weekly Hours20Pay RangeThe compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $123,800 - $170,300 per year. This job is eligible for a bonus incentive plan.
