At SYNERGY HomeCare of South Dayton, our mission is to bring compassionate, life-energizing care to individuals in the comfort of their own homes. We're looking for a caring and dependable Part-Time Licensed Practical Nurse (LPN) to join our team.

Responsibilities:
- Complete client assessments and reassessments
- Assist with care planning and documentation
- Monitor client health and communicate changes
- Provide nursing services within the LPN scope of practice
- Support caregiver training as needed
- Communicate professionally with clients, families, caregivers, and office staff
Qualifications:
- Current Ohio LPN license in good standing
- Current CPR certification
- Valid driver's license, reliable transportation, and auto insurance
- Strong communication and organizational skills
- Home care experience is preferred but not required
