The role involves conducting in‑person assessments to evaluate client eligibility and ongoing needs, contributing to critical care and support plans. Responsibilities Conduct UAS assessments for clients to determine level of care and support needs. Utilize the Uniform Assessment System (UAS) tool to document assessment results accurately. Provide compassionate and professional interactions with clients in various community settings. Submit completed assessments within designated timeframes and maintain detailed records. Collaborate with other care providers to ensure a comprehensive assessment process. Attend required training sessions and stay informed of assessment updates. Qualifications Registered Nurse, currently licensed and registered to practice in New York State. UAS‑NY Certified. Bachelor’s degree in nursing preferred. Experience in Home Care, case management, or long‑term home care. Demonstrated skills in planning, organizing, and directing the work of others. Experience in Intake or an admission team of PACE highly desirable, including completion of UAS‑NY Assessments. Proficient with DOH UAS assessment instrument.
