Licensed Social Service Director

Job Summary:

Ridgewood Rehabilitation & Living Center, a premier long-term care and rehabilitation facility, is seeking a compassionate and experienced Licensed Social Services Director to join our leadership team. As a key member of our interdisciplinary team, you will play a vital role in supporting residents and their families by addressing psychosocial, emotional, and discharge planning needs, while advocating for resident rights and promoting the highest level of independence and well-being.

Key Responsibilities:

• Complete admission assessments and comprehensive psychosocial evaluations

• Develop individualized care plans to address residents' emotional, social, and psychosocial needs

• Coordinate discharge planning and assist with safe transitions of care

• Provide support and counseling to residents and families throughout their stay

• Serve as an advocate for resident rights while promoting dignity, independence, and quality of life

• Collaborate with the interdisciplinary team to ensure comprehensive resident-centered care

• Maintain accurate and timely documentation in accordance with state and federal regulations

• Participate in care plan meetings, quality assurance initiatives, and interdisciplinary team meetings

• Maintain knowledge of community resources and assist residents and families with accessing available services

• Ensure compliance with all CMS, state, and facility regulations related to social services

Qualifications:

• Bachelor's degree in Social Work or a related human services field required (or qualifications meeting state regulations)

• Prior experience in long-term care or skilled nursing preferred

• Knowledge of Medicare, Medicaid, discharge planning, and long-term care regulations

• Strong communication, organization, and interpersonal skills

• Compassionate, resident-focused, and able to work collaboratively with an interdisciplinary team
