FHCSD is currently seeking a Physician to provide Post-Acute Care in an outpatient setting. This position will focus on supporting people who have been recently discharged from inpatient care and patients that have urgent care needs that are not appropriate for the ER. This role does not entail a primary care patient panel following. This is a unique opportunity to help underserved members of our community improve their long-term health outcomes while working as part of a care management team to address immediate needs.

• $290,000 - $335,000 Full Time Annually (part time and per diem also available)

Duties and Responsibilities

\n

\n
• Assess, plan and treat patient needs within an acute care setting
\n
• Triage patients to differentiate between immediate and less urgent care
\n
• Provide high quality and caring medical services to a diverse patient population
\n
• Perform routine exams, common procedures and assessments
\n
• Order and interpret laboratory tests, imaging, and point-of-care tests
\n
• Evaluate patients with complex problems and refer to appropriate specialists
\n
• Exercise cultural sensitivity and seek assistance with translations if needed
\n
• Complete all required documentation in accordance with departmental standards
\n
• Comply with electronic medical record requirements, including medication reconciliation, finalizing notes, clearing out personal pool, and participating in clinic pool
\n
• Attend all mandatory meetings and minimize incident reports
\n
• Use resources efficiently and practice medicine in a cost-effective manner
\n
• Participate in Clinical Outcomes Program
\n
• Maintain behavioral standards and respectful interactions with colleagues and patients
\n
• Obtain good peer reviews, attend mandatory meetings and minimize incident reports
\n
• Work toward meeting organizational targets for all metrics
\n
• Participate in administrative duties and on-call schedule in a timely manner
\n
• Must keep medical license, DEA registration and BLS/CPR certification current
\n
• Perform other duties as assigned

Requirements:

\n

\n
• Doctorate from an accredited school of medicine
\n
• Board certified or board eligible in a primary care field
\n
• Current unrestricted license to practice in the State of California or ability to obtain
\n
• DEA license
\n
• Current American Heart Association health care provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training
\n
• Experience with underserved, low-income, or homeless community members preferred
\n
• Second language fluency in desired
\n
