About the Role
As a Nurse Care Manager, you'll be responsible for coordinating care and developing individualized plans to ensure optimal health outcomes for our patients. You'll collaborate with interdisciplinary teams to identify and address patients' needs, and provide education and support to patients and their loved ones. With a focus on excellence, you'll work to deliver high-quality care that exceeds patient expectations.
NCM RN Nurse Care Manager Responsibilities
• Coordinate care plans with patients, families, and healthcare teams to ensure seamless transitions and optimal outcomes
• Conduct initial and ongoing assessments to identify patient needs and develop individualized care plans
• Collaborate with interdisciplinary teams to address patient concerns and develop solutions
• Monitor patient progress and adjust care plans as needed to ensure optimal health outcomes
• Maintain accurate and timely documentation of patient records
NCM RN Nurse Care Manager Requirements
• Current NY State RN license
• Strong communication and interpersonal skills
• Ability to work in a fast-paced environment with multiple priorities
