Experience: Minimum of 12 months of home health experience within the past 3 years

License: Current Physical Therapist license

Certifications: HCHB and OASIS experience required

Must-Have: Minimum productivity expectation of 32 points per week

Description:

Job involves providing physical therapy services in a home health setting.
