The ideal candidate will have strong experience in geriatric social services, care planning, case management, and supervisory leadership and a proven ability to deliver compliant, person-centered services that improve quality of life for veterans and their families .
Responsibilities:
- Plan, organize, and supervise the social services program for the Arizona State Veteran Home.
- Ensure timely completion of assessments and documentation in accordance with federal and state regulations and facility policies.
- Oversee compliance with the PASRR (Preadmission Screening and Resident Review) process.
- Supervise, coach, and evaluate Social Services staff through one-on-one sessions and annual performance appraisals.
- Plan, organize, assign, and review staff work to meet unit goals and objectives; address performance issues and recommend disciplinary actions as needed.
- Interview and select staff; promote team collaboration and professional development.
- Oversee clinically appropriate discharge planning and communicate plans to residents, families, and stakeholders.
- Monitor Medicare notices to ensure timely delivery to beneficiaries per federal law.
- Complete consults and referrals to specialists at the Veterans Administration (VA).
- Advocate for Resident’s Rights and ensure respectful treatment of veterans and members.
- Assist with department budgeting, operate within parameters, and monitor expenditures.
- Maintain a personal caseload and provide direct social work services.
- Participate in facility meetings, committees, training, and Arizona Management System (AMS) activities for continuous improvement.
Required Skills & Qualifications:
- Master’s degree in Social Work.
- Active social work license issued by the Arizona Board of Behavioral Health Examiners upon placement.
- Knowledge of CMS and State regulatory standards, PASRR, MDS system, long-term care billing, and community resources.
- Skills in counseling, case management, resident assessment, care planning, supervision, analysis, and problem solving.
- Ability to build relationships at all organizational levels, use facility IT systems, and maintain confidentiality.
Preferred Skills:
- One year of supervisory experience in a health care setting.
- Experience in long-term care and familiarity with geriatric psychiatric issues, disease processes, and psychotropic medication side effects.
