Job Summary

We are seeking a dedicated and experienced Home Health Registered Nurse to provide comprehensive, in-home patient care within Plymouth County, Iowa. This role is based out of Le Mars, Iowa, and focuses on delivering high-quality nursing services to patients in their residential settings.

The successful candidate will be responsible for managing a caseload of patients, conducting thorough assessments, developing individualized care plans, and ensuring continuity of care. This position requires a compassionate professional with strong clinical skills and the ability to work independently while collaborating with a multidisciplinary team.

Key Responsibilities

• Provide skilled nursing care to patients in their homes, adhering to established care plans and physician orders.
• Conduct comprehensive OASIS assessments to determine patient needs and eligibility for home health services.
• Develop, implement, and evaluate individualized patient care plans, adjusting as necessary to meet changing patient conditions.
• Monitor patient health status, administer medications, perform treatments, and educate patients and families on disease management and prevention.
• Maintain accurate, timely, and complete electronic health records in compliance with regulatory standards.
• Collaborate effectively with physicians, therapists, social workers, and other healthcare professionals to ensure coordinated patient care.
• Participate in the departmental call rotation, providing support for nights, weekends, and holidays.

Required Qualifications

• Active Registered Nurse (RN) license.
• Minimum of two (2) years of recent experience performing OASIS assessments and care planning in a home health setting.
• Ability to travel independently throughout Plymouth County, Iowa.

Preferred Qualifications

• Previous experience utilizing the Epic electronic health record (EHR) system.

Skills & Competencies

• Clinical Skills: Proficient in OASIS assessment, care planning, medication administration, wound care, IV therapy, patient education, and comprehensive physical assessment.
• Equipment: Competency with standard home health medical equipment.
• Soft Skills: Excellent verbal and written communication, strong organizational and time management abilities, critical thinking, problem-solving, empathy, adaptability, and the ability to work autonomously.

Shift Details

• Scheduled Shifts: Primarily days, Monday through Friday.
• Hours: 32-40 hours per week, typically 8-hour shifts, 5 days per week.
• Call Requirements: Required participation in a departmental call rotation, approximately one in four nights, weekends, and holidays.
• Contract Length: 13-week assignment.
• Start Date: July 28, 2026.

Licensing Requirements

• Active Registered Nurse (RN) license in the State of Iowa.
• Basic Life Support (BLS) certification.
