Job Requirements

• Active Registered Nurse license and eligibility to practice in Kentucky
• At least 1 year of nursing experience, preferably in home health or community health nursing
• Associate degree in Nursing or higher
• Strong time management, organization, and clinical documentation skills
• Ability to work independently in the field and adapt quickly to changing patient needs
• Willingness to learn the local service area and coordinate with field staff
• Comfort with on-call coverage, including select weeknights and periodic weekends

Responsibilities

• Complete skilled home health visits, including routine visits and OASIS assessments
• Provide high-quality, patient-centered nursing care in the home setting
• Coordinate care plans and communicate effectively with the interdisciplinary care team
• Manage daily visit schedules and travel between patient locations efficiently
• Support continuity of care across assigned counties
• Participate in on-call coverage as scheduled
• Document care accurately and maintain compliance with home health standards and regulations
