Client Care Coordinator

Title: Client Care CoordinatorLocation: New York City Metropolitan AreaReports to: Director of Legal ServicesClassification: ExemptEmployment Type: Full-time (35 hour per week)Role Summary: The Client Care Coordinator will work with clients in order to help them deal with issues involving their mental and emotional health as it pertains to their housing situation, which could include alcohol and substance abuse issues. In the context of mental health counseling, the Client Care Coordinator can have a huge impact on the individual during the course of their court case to ensure compliance and improve outcomesPrimary Job Responsibilities/Duties:The job duties of the Client Care Coordinator include, but are not necessarily limited to, the following:Providing emotional and mental health counseling to individuals facing eviction.Building strong rapport with clients and making accurate assessments of their problems whether mental/emotional or physical in the case of substance abuse.Responsible for assessing clients and referring clients to an appropriate agency for further assistance upon completion of the housing court case.Keeping detailed and concise records of each client during the course of their counseling.Provide effective treatment planning and assist clients in successfully maintaining their housing or being relocated to more appropriate housing.Evaluate crisis situations and apply appropriate interventions .Assist in the detection of “at risk” clients and development of plans to prevent further psychological or physical deteriorationProvide consultation and brief intervention for psychological/psychiatric problems and/or disorders.Actively participate in meetings that support the positive outcome of the client’s court case.Develop and maintain liaison relationships with community resources, specifically other behavioral health and substance abuse agencies for possible referrals of clients needing long term care.Provide support to legal staff with difficult clients by reaching out to said clients via phone, letter, email or in person home visits to determine why the client is not communicating with the staff.Participate fully in legal departmental meetings and other agency meetings as warranted.Create a monthly newsletter for the staff.Assist in departmental tasks, including programmatic and infrastructure design, implementation, and evaluation.Ability to work in a stressful setting and assist clients facing eviction with maintaining their housing by providing the client services to enable a positive outcome of their case.Conduct home visits to assess client and determine if referrals to outside agencies are needed and determine best case plan to assist tenant in achieving positive outcome with their court case.Physical Requirements:Ability to walk, stand, kneel, crouch, and bend over.Ability to lift, reach, and handle objects weighing up to 25 pounds.Ability to sit at a desk and work on a computer for prolonged periods.Ability to travel to different locations within New York City, as needed.Work Environment / Schedule Requirements:Office setting with regular exposure to computer screens and moderate noise levels.May be required to work on or during evenings, weekends, and holidays.Qualifications:Masters in Social Work.Strong interpersonal skills with the ability to be compassionate and firm and maintaining confidentiality at all times.Knowledge of community resources.Ability to solve problems, make decisions, resolve conflicts and listen.Ability to deal calmly in crisis situations.Proven ability to work collaboratively well with diverse groups.Proven ability to handle multiple tasks effectively under pressure.Strong organizational skills, detail-oriented, and efficient.Maturity, integrity, and sound judgment.Bilingual Fluency (Spanish/English) is preferredMust be able to read, speak, write, and understand English for administrative purposes.Must pass drug screening to be appointed.This position may be subject to a series of investigations before and after appointment.Equal Employment OpportunityNAICA is an equal opportunity employer committed to maintaining a diverse workforce and inclusive work environment.
