We are seeking a part-time Care Coordinator for an on-site role in Houston, TX, supporting client care coordination with a focus on case management and patient advocacy. This position is intended for a professional who can work closely with individuals navigating healthcare, housing, and support services, while coordinating care plans and connecting people to appropriate resources. The client is looking for someone with a bachelor's degree and prior case manager experience, along with strong communication, collaboration, and leadership skills. Experience supporting individuals with SSI, housing opportunity assistance, homelessness, substance use, and mental health needs is especially relevant for this role. Comfort working with people who may have poor hygiene is also important in this environment.

Benefits

• Weekly pay range of $18.85 to $20.00, based on the stated compensation structure.
• Takeover option by client, subject to client discretion and role suitability.

What You Will Do?

• Coordinate care plans for individuals and ensure services are aligned with their identified needs.
• Support case management activities by tracking progress, documenting updates, and following established procedures.
• Advocate for patients and clients while helping them access healthcare and community resources.
• Navigate healthcare and support systems to connect individuals with appropriate referrals and assistance.
• Collaborate with internal teams and external partners to maintain consistent and effective care coordination.

Your Skills

• Demonstrated leadership skills to help guide care coordination efforts and support client-focused solutions.
• Experience with care plan coordination in a case management or related healthcare support setting.
• Strong patient advocacy skills to represent client needs and promote appropriate services.
• Knowledge of healthcare navigation, including referrals, support programs, and community-based resources.
• Clear communication and collaboration skills for working with clients, families, and multidisciplinary teams.
• Proficiency in English to support professional communication, documentation, and client interaction.

Pay Details: $18.85 to $20.00 per hour
