Care Management Coordinator

We're searching for a Coordinator of Respiratory Care -- someone who works well in a fast-paced setting. In this position, you will demonstrate clinical expertise in the physical and educational aspects of Respiratory Care for the pediatric and neonatal patient population and their families as outlined in the unit profiles. The CRS is dedicated to promoting optimal patient/family care with the ultimate goal of enhanced patient/family outcomes. Serves as an advanced clinical practitioner. Collaborates with the health care team to assist in the planning, implementing, and evaluating of treatment approaches utilizing clinical knowledge, sound judgment, and problem-solving. For example, participation in interdisciplinary rounds offering a Respiratory Care perspective to patient care; evaluating response to interventions and modifying plan of care; Acts as a clinical resource to staff and other members of the health care team. For example, available by pager or phone when out of the unit; approached by staff and other members of the health care team regarding clinical issues in the area of expertise. • Maintains clinical skills by providing direct patient care and related activities. Performs patient care and related activities within the scope of their training and responsibilities (demonstrating advanced clinical competencies in an area of expertise). • Serves as an educational resource in expertise for the patient and family, and other health care professionals. Conducts research and development activities for Respiratory Care. • Serves as an administrator. Required Technical diploma Respiratory certification/registry program. Preferred Associate's Degree or Bachelor's degree Respiratory Care.
