ABOUT THIS POSITION
The Department of Family Services (DFS) Adult and Aging Division welcomes applicants for the Public Health Nurse III (PHN III) position. The role oversees the Nurses II unit, ensuring the DFS values are integrated into staff operations and services. Responsibilities include clinical supervision and support to Family Safety Practitioners (FSPs) and Public Health Nurses (PHNs) involved in Adult Family Services and Adult Protective Services (APS), ensuring compliance with local, state, and federal regulations. The supervisor mentors PHNs and specialists in assessment, care planning, and service delivery aligned with a client‑centered, least‑restrictive approach. Duties also involve interpreting and drafting local policies related to LTSS assessments, guiding staff on policy and best practices in case management, home care services, CCC Plus screenings, and long‑term care Medicaid, onboarding new employees, ongoing coaching, field visits, monitoring case outcomes, maintaining client files, managing expenditures, and maintaining daily workload distribution. The role collaborates with community and other county agencies, adapts to changing priorities, promotes the agency’s mission, and addresses client and community concerns. Coverage for the Assistant Program Manager may also be required. The position functions as a certified Long Term Services and Supports (LTSS) Screener, conducting home visits, assessing clients’ health and safety needs, and determining levels of care per DMAS guidelines. The PHN III applies a person‑centered approach, effective communication, critical thinking, and cultural agility while respecting client preferences. Other activities include outreach, health promotion, and education, where applicable.RESPONSIBILITIES
Serve as a certified LTSS Screener with a multidisciplinary screening team. Consult on screenings and all other Adult and Aging inquiries. Conduct home visits and determine clients’ health and safety needs and level of care per DMAS guidelines. Conduct field visits to a diverse population, including at‑risk, medically fragile, disabled seniors, adults, and children. Use a person‑centered practice, cultural agility, engaging communication, and critical thinking skills. Work independently and collaboratively, respecting others’ opinions and clients’ choices regarding their plan of care. Make appropriate referrals and recommendations. Implement public health nursing activities, including outreach, case findings, health promotion, education, care coordination, and disease prevention. Attend required meetings and participate in training. Assess risks posed to clients. Interpret state and local policy; mentor and train staff; conduct staff training and provide educational presentations to community groups. Supervise mixed professionals, including PHNs, FSPs, and administrative support. Demonstrate assessment, care planning, and coordination skills; manage competing priorities; respond effectively to crisis situations.REQUIRED KNOWLEDGE, SKILLS AND ABILITIES
Knowledge of public health nursing theories, principles, and practices to provide quality patient care. Knowledge of human growth, development, anatomy, physiology, pharmacology, preventive medicine, epidemiology, and nutrition. Knowledge of current practices, literature, and trends in public health nursing, including environmental, economic, psychological, social, cultural, and linguistic issues. Knowledge of documentation, nursing diagnosis, and quality assurance. Knowledge of medical and legal policies, guidelines, principles, practices, and standards governing public health nursing. Extensive knowledge of community resources for referral and follow‑up. Ability to interpret and apply professional standards, county health department policies, and applicable laws and regulations. Ability to build and maintain partnerships with healthcare providers. Ability to relate well to clients from varied backgrounds. Ability to manage or diffuse difficult or distressed client or staff situations. Ability to identify problems and make sound, well‑informed decisions. Ability to assess the quality and impact of decisions by others. Ability to provide health training to clients with complex needs. Ability to use technology to retrieve and analyze data and communicate public health information. Ability to implement public health nursing programs. Ability to advocate for changes in the county’s public health care delivery system. Ability to supervise the work of professionals, paraprofessionals, volunteers, and students. Ability to develop, plan, and coordinate services with other health disciplines.EMPLOYMENT STANDARDS – MINIMUM QUALIFICATIONS
Graduation from a college or university accredited by the National League for Nursing or the Commission on Collegiate Nursing Education with a bachelor’s degree in Nursing; plus three years of post‑licensure experience as a nurse in a primary prevention or community education setting, including two years in a public health or community health setting; plus at least one year of experience in a public health/community specialty area or supervisory responsibilities.CERTIFICATES AND LICENSES REQUIRED
Current license or multistate licensure privilege to practice as a Registered Nurse in Virginia. Valid driver's license. AED certification required within 60 days. CPR certification required within 60 days.NECESSARY SPECIAL REQUIREMENTS
The appointee must complete a criminal background check, Child Protective Services Registry check, driving record check, and sanction screenings to the satisfaction of the employer. The position is emergency/essential services personnel; required to report for duty during inclement weather and/or other emergencies.PREFERRED QUALIFICATIONS
Certified LTSS Screener. Working knowledge of community‑based and long‑term care services. Skill in basic and advanced health care assessments. Proficient interviewing skills in collecting medical histories. Experience with diverse populations, including at‑risk, medically fragile, and disabled seniors, adults, and children. Familiarity with DMAS regulations and Virginia Department of Health code regarding LTSS. Ability to assess homes for adaptive changes and facilitate client care changes. Strong critical thinking and problem‑solving skills. Exceptional interpersonal and communication skills, both oral and written. Model outstanding customer service skills and build effective relationships. Proficient computer and software skills: Microsoft Office Suite, virtual meetings, electronic signing, and electronic medical record systems. Good oral and written communication skills. Good organizational skills. Experience using Microsoft Word, Excel, Access, and Outlook.PHYSICAL REQUIREMENTS
Ability to assist with client transfers, drive a vehicle, walk, stand, sit, bend, kneel, reach, climb stairs, and carry equipment or small children up to 25 pounds. Frequent communication, accurate information exchange, hand use, and visual acuity to read data on a computer monitor are required. Ability to operate keyboard‑driven equipment.
