What You'll Do

• Perform patient evaluations including history, systems review, and specific tests
• Develop individualized care plans targeting pain reduction and functional restoration
• Conduct ongoing reassessment of patient progress and adjust treatments accordingly
• Coordinate discharge planning from the start of care
• Supervise and delegate tasks to ancillary staff and students in compliance with state laws
• Document all care accurately and timely per policies and regulations
• Communicate effectively with patients, families, caregivers, and healthcare team
• Educate patients and families on treatment plans and health management
• Participate in ongoing professional development and quality improvement activities

What You Need

• Valid state Physical Therapy license
• Prior experience in long-term care or related environments preferred
• Strong assessment, communication, and teamwork skills
• Ability to perform detailed evaluations and develop effective treatment plans
