Bilingual Medical Coordinator
The Medical Coordinator for La Jornada arranges and coordinates the proper delivery of medical, dental, mental health, and pharmacies services on behalf of children in care. This includes urgent, emergent and preventive health care. The Medical Coordinator accurately documents the services provided by maintaining complete and confidential records for children.
The Medical Coordinator regularly communicates with children's care teams and coordinates appointments and interpretation services, and strives to meet continuous quality improvement measures. The Medical Coordinator provides children with a positive medical environment.
Responsibilities include performing all work in a manner consistent with the National Youth Advocate Program's mission, values, and philosophies; providing children with a positive and appropriate medical environment; ensuring confidentiality of health records; working to obtain Memorandum of Understandings with medical, dental, and mental health providers; participating in intakes and discharges of children; continuously assessing the children's medical, dental, and mental health needs; collaborating with children's assigned cross-functional team and medical, mental health, dental, and pharmacy providers; reporting medical emergencies; elevating issues of medical concern; supporting foster parents; collecting forms and health records; completing and submitting all assigned documentation; maintaining medicine cabinets and first aid kits; providing in-service training and ongoing education; maintaining Red Cross CPR/First Aid Trainer's Certifications; assisting and supporting Program Leadership during auditing processes; and performing other duties as requested.
Minimum qualifications include a Licensed Practical or Vocational Nurse (LPN/LVN) or higher health-related qualification or a Bachelor's degree in a relevant field; bilingual fluency in English and Spanish; minimum of 1 year of employment in public health or a healthcare setting with demonstrated experience in care coordination, health administration, or direct clinical care; (1) year of experience working in the child welfare field working with children and/or adolescents in a social service setting; a valid driver's license, excellent driving record, a safe and clean personal vehicle, and current automobile insurance; and the ability to maintain compliance with attending agency required training programs. Proficient use of desktop and laptop computers, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs is also required.
Other skills include experience scheduling medical appointments, experience documenting medical appointments, recognizing and understanding the needs of the immigrant population, providing support and creating a safe environment for the children in the program, and demonstrated ability to maintain liaisons with community health providers and expand community health network. Must be open to and willing to work non-traditional hours and be available for on-call phone consultations as needed.
Physical demands include use of manual dexterity, tactile, visual, and audio acuity; use of repetitive motion, prolonged periods of sitting and standing, and sustained visual and mental applications and demands; occasional lifting (up to 25 pounds), bending, pulling, and carrying; and ability to travel frequently and drive vehicle while sitting for extended periods, with frequency varying based on program demands.
