Key Responsibilities\n

• Design, coordinate, and implement individualized support plans aimed at addressing clients' personal challenges and dependencies.

• Conduct comprehensive assessments of clients, including interviews, to evaluate their current psychological status, needs, strengths, and areas for growth.

• Establish specific and measurable goals for each client, tailoring intervention strategies to enhance their coping abilities.

• Continuously monitor and assess clients' progress, making adjustments to treatment plans as necessary to ensure optimal outcomes.

• Provide information and guidance during counseling sessions, empowering clients to make informed decisions regarding their care.

• Maintain thorough and accurate documentation of client interactions, interventions, and progress reports to facilitate ongoing support and accountability.

• Participate in case conferences and, when necessary, provide testimony in legal proceedings to advocate for clients’ needs.

• Serve as a primary case manager, coordinating with a team of healthcare professionals to ensure integrated care for clients.

• Facilitate referrals to community resources and support services that will aid in clients' recovery and progression.

Patient Population / Client Focus\n

Your role will primarily focus on supporting clients facing complex emotional, social, and psychological challenges related to end-of-life care. This includes individuals with terminal illnesses and their families, requiring a deep understanding of grief, bereavement processes, and the social dynamics at play during such critical life transitions.

Qualifications\n

• A valid and active license as a Clinical Social Worker (LCSW) in Florida.

• A Bachelor's degree in Social Work, Psychology, Sociology, or a related field is required; a Master's degree is preferred.
