Nurse Navigator - Ambulatory Neurology (18‑Month Grant) FT

Henry Ford Health System in Detroit is seeking a Nurse Navigator for an 18-month term grant-funded position focusing on Alzheimer’s disease care coordination. You will assist with the coordination of care for patients undergoing evaluation and treatment for cognitive impairments. The ideal candidate will have extensive experience in ambulatory nursing and care coordination. Full-time work is Monday to Friday, with no weekends or holidays.
