Schedule Opening:

• Will cover both Eau Claire locations along with our Osseo location
• flexible schedule, including days and hours
• weekends and holidays are infrequent/limited
• continuing education assistance

Responsibilities:

• plan, develop, organize, implement, evaluate, and direct physical therapy, as well as its programs and activities, in accordance with current rules regulations, and guidelines that govern a skilled nursing facility
• participate in discharge planning, development and implementation of patient care plans, patient assessments, etc. as necessary
• maintain treatment records, patient files, and progress notes as required
• participate in the development and implementation of patient assessments and care plans, including quarterly reviews

Required Qualifications:

• current license as a physical therapist in the State of Wisconsin
