Job DescriptionJob DescriptionSummary: The Professional Level Service Provider is responsible for providing clinical case management services to each child in care.
Any additional information you require for this job can be found in the below text Make sure to read thoroughly, then apply. Essential Duties and Responsibilities:1. Completing an admission assessment or any other evaluation of a child for placement.
2. Developing, reviewing, and updating of service plans for a child in care.
3. Completing a discharge or transfer summary for a child.
4. Approving any restrictions that will be imposed on a child for more than seven days that have not been reviewed and approved by the treatment director or service planning team, and any monthly re-evaluations of restrictions that continue for more than 30 days.
5. Approving any restrictions to communication and visitation with the childs family that are imposed on a child, but have not been reviewed and approved by the treatment director or service planning team, including monthly re-evaluations of restrictions that continue for more than 30 days: and
6. Approving any restrictions to a particular room or building for more than 24 hours that are imposed on a child but have not been reviewed and approved by the treatment director or service planning team.
7. Working closely with the Treatment Director, Therapists, Case Managers, and medical staff to ensure treatment plans are effective, thorough, and accurate for that childs treatment needs.
8. Attends and participates in all Service Planning, Discharge, Psychiatric Medication Evaluations, EBI, staffing, and departmental meetings.
9. Attend ARDs for children who require special education services and attend any other scheduled meetings regarding the childs educational needs.
10. Assist in psychiatric hospitalizations when needed.
11. Gathers and reviews all information important to the residents treatment plan, i.e. medical appointments and documentation, and educational information.
12. Ensure that all contact with the children is documented on a case note and added into that childs treatment plan.
13. Maintaining accurate, current, and complete documentation for each childs record and filed in a timely record.
14. Utilize CANS Assessment (The Child & Adolescent Needs and Strengths) tools to facilitate quality improvement initiatives and monitor the outcomes.
15. Perform all duties within contract, state and federal regulations, Kismet policy, and COA standards.
16. Perform other job duties that may be assigned by the executive director or department head.
17. Assist in psychiatric hospitalizations when needed.
18. Complete Ansell Casey Life Skills Assessments for all children who require PALS
19. Coordinate PALS Services for all children 16yrs and older.
20. Attend all Circle of Support meetings for each child.
21. Assist and prepare all children 16yrs and older in transitional living and life skills. (i.e. completing FASA, career interest, housing, money management, TXID, vocational training, transportation, medical services, psychiatric services, public assistance, etc.)
22. Perform monthly audits of each childs chart and educational portfolio. Assemble educational portfolios for children who have not received an educational portfolio within 30 days from their admission date.
23. Provide resource information and referrals to community organizations for in-care and after-care therapeutic treatment such as behavior modification, stress management, therapy to assist children in controlling disorders and other problems to the child and their managing conservator. Maintain a comprehensive up-to-date list of community programs, services and information on how to access them for each child.
24. Provide consultation for direct care staff, probation officers, caseworkers, medical providers, family members and other individuals as indicated by the childs treatment plan.
25. Consult with the outside specialists and treatment team members concerning the immediate needs of each child when needed. 26. Attend department meetings, seminars, additional trainings, and professional conferences.
27. Update each childs approved contact lists, phone call logs, managing conservator contact information and placement authorizations. Update copies of all approved contact lists in each childs chart and provide copies in the phone call binders for each house. Update each childs call logs monthly.
Also, must have: The ability to relate positively to young people with serious behavioral and emotional disturbances.
Possess or be able to complete certification in basic First Aid, CPR, and restraint training.
Ability to safely accomplish, with or without assistance, physically and emotionally demanding containment of a resident, when necessary, using proper techniques.
Competent in managing aggressive, verbally abusive, and sexually inappropriate behaviors attributed to some residents illnesses.
Working knowledge of licensing standards and contract guidelines applicable to program.
Possess sensitivity to the service population's cultural and socio-economic characteristics.
Ability to establish a respectful relationship with persons served in order to help them gain skills and confidence.
Ability to work collaboratively with other staff members, service providers and professionals.
Capacity to maintain a helping role and to intervene appropriately in order to meet service goals.
Demonstrate a commitment to empowering others to solve their own problems.
Demonstrate a conviction about the capacity of people to grow and change.
Possess excellent interpersonal skills, as well as organizational and planning skills.
Possess excellent communication skills both verbally and in writing.
Computer literate and able to work with computer programs.
Punctuality is required when meeting deadlines and reporting to work. SUPERVISORY RESPONSIBILITIES
N/A Education and/or Work Experience Requirements:
A masters degree or higher from an accredited college or university in social work or other human services field and nine credit hours in graduate level courses that focus on family and individual function and interaction. One year of documented full-time work experience in a treatment setting serving children, including RTCs, child-placing agencies providing treatment services, and psychiatric hospitals serving children.
Must obtain the required annual training hours mandated by the state.
Must complete the CANS Assessment within 6 months of hire

Ability to apply common sense understanding to carry out simple one or two-step instructions. Ability to deal with standardized situations with occasional variables. OTHER REQUIREMENTS
Valid driver's license, appropriate insurance and reliable car. Must submit to and pass a pre-employment drug test.
Must submit to and pass random drug tests during employment.
