Description Job Responsibilities Provide patient-centered, trauma-informed care, for individuals experiencing homelessness residing in recovery residences. Lead wound care (xylazine wound experience preferred), complex care management, harm reduction education, and general primary care needs. Document patient encounters in data collection system and electronic health record. Work collaboratively with an interdisciplinary care team including physicians, care managers, substance use navigators and certified recovery specialists, etc. Other duties and responsibilities as assigned Qualifications Bachelor's degree and 5-7 years of experience along with Registered Nurse with Licensure in Commonwealth of Pennsylvania without restriction or limitation required. BSN preferred. Bilingual (Spanish) preferred.
