- Help residents improve mobility, strength, balance, and independence
- Collaborate with nursing and care teams to support resident outcomes
- Educate residents and families on treatment goals and progress
- Maintain accurate OASIS documentation and therapy records
- Active Pennsylvania physical therapist license required
- Experience in home health or short-term rehabilitation is preferred but not required
- Strong communication and problem-solving skills
- Ability to work independently and as part of a team
- Passion for helping seniors achieve their highest level of function
