Home Physical Therapist (PT), Occupational Therapist (OT) & Speech-Language Pathologist (SLP)

Life and Light Healthcare Services is seeking compassionate, dependable, and skilled Physical Therapists (PT), and Occupational Therapists (OT) to provide high-quality, patient-centered therapy services in patients' homes throughout Maryland.

We are looking for dedicated clinicians who are passionate about helping patients improve mobility, function, communication, independence, safety, and overall quality of life while receiving care in the comfort of their homes.

Position Overview

The Home PT, and OT, will provide skilled evaluation, treatment, education, and ongoing assessment based on each patient's individual needs and established plan of care.

The clinician will collaborate with patients, families, caregivers, physicians, nurses, and other members of the interdisciplinary healthcare team to promote safe, effective, and coordinated patient care.

Essential Responsibilities

• Perform comprehensive initial and ongoing evaluations and functional assessments.
• Develop and implement individualized treatment plans within the clinician's scope of practice.
• Provide skilled therapy services in accordance with the patient's plan of care.
• Monitor and document patient progress and response to treatment.
• Modify treatment approaches as clinically appropriate and within applicable professional requirements.
• Provide patients and caregivers with education, training, and appropriate home exercise or therapy programs.
• Promote patient safety, independence, mobility, communication, and functional abilities.
• Identify changes in patient condition and promptly communicate significant concerns to the appropriate healthcare professional.
• Coordinate care with nursing, physicians, caregivers, and other interdisciplinary team members.
• Complete evaluations, treatment notes, reassessments, progress reports, recertifications, and discharge documentation accurately and within agency requirements.
• Maintain timely and accurate documentation in the agency's electronic health record.
• Follow applicable federal and Maryland laws, regulations, professional standards, and agency policies and procedures.
• Follow infection prevention, patient safety, privacy, confidentiality, and HIPAA requirements.
• Participate in quality improvement, care coordination, case conferences, and clinical care activities as applicable.
• Maintain professional boundaries and provide respectful, culturally sensitive, and patient-centered care.

Qualifications

• CPR/First Aid
• Current, active, and unrestricted Maryland license in the applicable discipline:
• • Physical Therapist (PT)
  • Occupational Therapist (OT)
• Graduate of an accredited educational program in the applicable discipline.
• Current professional credentials and certifications required for the position.
• Home health experience is preferred but not required .
• Strong clinical assessment, communication, documentation, organizational, and time-management skills.
• Ability to work independently and exercise sound clinical judgment in patients' homes.
• Reliable transportation and ability to travel to assigned patient locations.
• Ability to successfully complete agency-required background screening, credential verification, employment verification, and onboarding requirements.

Employment & Schedule

Employment Type: PRN or On-Call

Schedule: Flexible scheduling based on patient needs and clinician availability.

Setting: Patients' homes

Service Area: Maryland

Travel: Travel between assigned patient homes is an essential function of this position.
