Park Place Transitional Care & Rehab is hiring a Physical Therapist for our in-house, close-knit therapy team. We treat ortho, cardiac, neuro and wound care patients, plus our ventilator unit. With Montana's first long-term ventilator and tracheostomy unit on site, you can build high-acuity respiratory skills without leaving the state.

• Screen and evaluate patients with physical functioning disorders and/or wound care needs and develop a plan of care that meets regulatory and clinical standards

• Provide skilled, medically necessary treatment, including manual techniques, ambulation, therapeutic exercise, modalities, assistive devices and wound care

• Assess progress, adjust treatment and document in the clinical record

• Counsel patients, families and caregivers

• Communicate progress, barriers and plans with your supervisor and the team

• Complete a full discharge summary

• Licensed Physical Therapist (Montana or eligible)
