Major duties include but are not limited to:

• Delivery of care is in accordance with the established standards, policies, and procedures consistent with the interdisciplinary plan of care.
• Provides consultation and guidance and acts as a liaison to staff, nursing homes, and other health care providers regarding patient needs in the community/home setting and in the interpretation of VA rules and regulations.
• Assesses and charts observations of the veteran's condition at each visit.
• Completes evaluation tasks that include medication review, vital signs, wound care, and other duties within the RN scope.
• Administers medication as prescribed by provider.
• Coordinates plan of care with providers, therapists, and other interdisciplinary team members.
• Willing to work cooperatively as a member of a team in all assignments.
• Makes caring for the veteran his/her priority while in the work setting and demonstrates customer service principles in all aspects of work.
• This position RN may be required to perform home health aide tasks and homemaking tasks if indicated.

Travel Requirement: You may be expected to travel up to 75% of the time for this position.

Drivers License Requirement: Unrestricted drivers license is required. Must provide a clean driving record report from the DMV during onboarding.

Physical Examination: A driver's physical is required for this position.
