This new senior community is in a beautiful and serene location atop the bluffs of Dubuque, Iowa, overlooking the Mississippi River. This community offers the following care options: Residential Care Facility (RCF), Residential Care Facility (RCF) with Memory Care, Long Term Care/ Care Center Address: 1160 Carmel Dr, Dubuque, IA 52003 Near by towns include: Bellevue, Cascade, Peosta, Maquoketa, Platteville, WI, and Galena, IL. Hiring Range: $39.28 - $48.53/hr (based on experience) Responsibilities As a Physical Therapist you will be responsible to: Conduct a comprehensive assessment & evaluation using subjective and objective measures. Develop treatment plans and programs in accordance with physician orders. Provide quality treatments, actively progressing goals to achieve highest quality of life. Conduct screens and provide education to residents/clients, families, caregivers, and staff. Coordinate seamless discharges utilizing the full continuum of therapy and fitness services. Provide accurate and timely documentation & billing in electronic medical record. Demonstrate understanding and knowledge of payer sources and regulatory requirements. Complete and document supervisory visits per regulations. Communicate effectively with residents/clients, families, caregivers, physicians, referral sources, and interdisciplinary team. Qualifications Graduate from an accredited Physical Therapy program. Current licensure in the state(s) of practice as required. Demonstrated compatibility with PHS's mission and operating philosophies. Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.
