At Bradford Manor Nursing and Rehab we pride ourselves on the exceptional care we provide for both our residents AND our staff. We are seeking dedicated Speech Language Pathologist (SLP) to join our outstanding and growing team!
Key Responsibilities:
Patient Assessment: Perform comprehensive evaluations of patients’ speech, language, cognitive-communication, and swallowing abilities.
Treatment Planning: Develop individualized treatment plans based on assessment findings and patients’ needs, with a focus on achieving measurable, functional goals.
Intervention and Therapy: Provide therapy interventions for speech, language, voice, fluency, cognitive-communication, and swallowing disorders. Utilize evidence-based approaches tailored to the SNF environment.
Collaboration: Work closely with an interdisciplinary team, including physical therapists, occupational therapists, nurses, and physicians, to ensure holistic and coordinated patient care.
Documentation: Accurately document patient progress, treatment interventions, and outcomes in accordance with regulatory standards and facility policies.
Family and Caregiver Training: Educate patients, families, and caregivers on techniques and strategies to support communication and swallowing function outside of therapy sessions.
Compliance: Ensure all treatment and documentation adhere to Medicare, Medicaid, and facility guidelines.

Requirements:
Licensed as a SLP in PA
Valid Certificate of Clinical Competence (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA).
1+ year experience in a Skilled Nursing Facility
Compassion and patience when working with elderly or disabled patients.
Strong clinical reasoning, communication, and interpersonal skills; proficiency with electronic medical records (EMR) preferred.
