What You'll Do

• Evaluate and rehabilitate patients with physical functioning disorders or wound care needs
• Develop and modify individualized care plans using standardized assessments and evidence-based practices
• Perform hands-on procedures, therapeutic exercises, use of assistive devices, and wound care
• Monitor patient progress and adjust treatments accordingly
• Document patient evaluations, treatment sessions, and outcomes accurately in electronic records
• Provide consultation to patients, families, and care teams regarding treatment plans
• Support supervision of rehab assistants and support staff as needed
• Participate in team meetings, in-services, and training sessions

What You Need

• Valid Physical Therapist license for Oklahoma
• Minimum of 1 year of relevant experience in LTC or outpatient settings
• Strong communication and teamwork skills
• Ability to work independently and adhere to clinical standards
