Care Manager Assistant Job Summary: At BrightCare HomeCare, we are dedicated to providing exceptional home care services to individuals with disabilities, injuries, and illnesses in the comfort of their own homes. We are seeking a compassionate and organized Care Manager Assistant to join our team in Baton Rouge, LA. As a Care Manager Assistant, you will play a crucial role in supporting our care managers in coordinating patient care, ensuring seamless transitions, and promoting optimal health outcomes. Responsibilities: Assist Care Managers in coordinating patient care, including scheduling appointments, managing medical records, and communicating with healthcare providers Ensure accurate and timely documentation of patient information, including assessments, progress notes, and care plans Collaborate with interdisciplinary teams to develop and implement personalized care plans, addressing patients' unique needs and goals Provide support and guidance to patients and their families, addressing concerns and questions, and promoting education and awareness Maintain accurate records of patient contacts, including phone calls, visits, and emails Participate in quality improvement initiatives, identifying areas for improvement and contributing to process enhancements Requirements: High school diploma or equivalent required; associate's or bachelor's degree in a related field preferred Prior experience in healthcare, social services, or a related field; minimum of 0 years of experience Excellent communication, interpersonal, and organizational skills Ability to work effectively in a fast-paced environment, prioritizing multiple tasks and deadlines Familiarity with electronic medical records and other healthcare software What We Offer: Full Benifits How to Apply: If you are a motivated and compassionate individual looking to make a positive impact in the lives of others, we encourage you to apply for the Care Manager Assistant position at BrightCare HomeCare.
