Responsibilities
- Conduct initial patient evaluations to determine PT needs for residents in a skilled nursing setting.
- Develop and implement individualized treatment plans with measurable, function-focused goals.
- Provide evidence-based interventions (e.g., therapeutic exercise, gait and balance training, functional mobility, manual therapy as appropriate).
- Reassess progress, document outcomes, and adjust plans to maximize functional recovery.
- Collaborate with an interdisciplinary team (nursing, occupational therapy, speech therapy, physicians) to coordinate care and progress toward discharge.
- Educate patients and families on home exercise programs, safety, and strategies to maintain gains after discharge.

Requirements
- Active physical therapist license in California
- Ability to work in a travel assignment and meet credentialing requirements
- Experience or comfort working in a skilled nursing facility/geriatric rehab setting
- Strong clinical documentation and communication skills
- Availability to work the stated shift and guaranteed hours (36 per week)
