Job Description

Seeking a full-time Resident Care Coordinator to work with our Care Director to assist with management and supervision of care services to our Assisted Living residents, based in the San Diego, CA area. This involves providing direct care, supervising, and educating the caregivers.

Work Schedule/ Shift

40 hour work week/ flexible schedule needed

Compensation

Starting at $24.00/hour

What You'll Do

  • Communicate professionally with the residents, families, and peers and, as part of a team, help to create a positive, caring environment.
  • Supervise the caregiving operations consistent with the organization's standards, mission, and goals.
  • Provide and evaluate the provision of services to the residents with appropriate regard for the resident's physical, mental and social well-being, including those with mild or moderate dementia.
  • Provide direct supervision to Resident Care personnel who maintain the daily environment of residential care.
  • Provide orientation, training, supervision, and evaluation of all Resident Care personnel.
  • Make daily rounds to monitor care provided to residents in the community.
  • May assist with hire, performance-manage and schedule staff as appropriate.
  • Assess current and prospective residents for their level of functioning and appropriateness for varying levels of care.
  • Create appropriate service plans for new and existing residents.
  • Participate within an interdisciplinary team which monitors and reviews residents' health and social needs on an ongoing basis.
  • Maintain contact with residents' health care providers, as needed, including reassessments prior to and coordination of services upon return from hospitalization.
  • Assure that physician's orders are clearly documented, clinical services provided, and health care issues are addressed.
  • Oversee the medication management program and assures that the appropriate medication and resident health records are maintained.
  • Establish and maintain a thorough knowledge of federal, state and local legislation and accrediting/contracting agency policy and procedures related to Residential Care.
  • Perform other duties as assigned.

Requirements & Qualifications

  • Minimum five to seven years of experience in senior living/ memory care, healthcare, or related industry.
  • Minimum two to three years of supervisory experience to include training and supervising staff and developing and managing a budget.
  • Bachelor's degree in nursing, human services, healthcare, or related fields or combination of education is preferred
  • Experience supervising others in a senior living/memory care or hospitality environment.
  • Completion of required specialized Nursing coursework for certification or certification (RN, LPN or LVN) is a plus
  • Adaptability and strong organization and time management skills.
  • Excellent resource management, computer, record keeping, interpersonal, decision making, and verbal and written communication skills.
  • Patience and compassion when dealing with others, especially those who are sick, injured, elderly, or from different backgrounds.
  • Flexibility to work when needed, especially evenings, weekends, or early mornings.
  • Willingness to adhere to and enforce all safety policies and procedures.

While performing the duties of this job, the employee must continually demonstrate ongoing sustained ability to perform these physical demands:

  • Prolonged periods of sitting at a desk and working on a computer.
  • Possible prolonged periods of standing and bending.
  • Must be able to lift up to 50 pounds at times.
