Physician - OB/GYN Fort Morgan

Summary: St. Elizabeth is looking for OB coverage for open dates starting Mar 2025. Ongoing dates will be TBD, as the facility completes their schedules. Site will put a preference on locums who are local to the facility due to some stints being 1 day.

Rate Caps for Submission:
Malpractice - Must be included in rates.
Orientation - *** - half the hourly/callback rate*** kicks in after 8 hours. "Per hour of orientation or training beyond 8 hours"
Holiday - Must be 1.5x the regular rate*** "Per holiday hour, no OT"
On Call (Daily) - *** - "Per 24 hours to include 4 hours gratis"
24 Hour Holiday Call Rate - *** - "Per 24 hours to include 4 hours gratis"
Call Back - *** - "Per hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms."
Holiday Call Back - *** - "Per holiday hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms."
Average Worked Callback Hours: 5 hours

Deal breakers: Candidate must be able to cover full Feb stint, no partial shifts will be accepted.


EMR Required Training:
Module (learn lessons) - Must be completed prior to virtual EMR training to attend the training.
EMR Virtual training - Only able to attend if Modules are completed before training. Virtual EMR training must be scheduled prior to the providers first day, to avoid the provider missing their first day.
All candidates must complete the following EMR questionnaire and detail it in the notes section of the candidates submission at time of presentation.

Travel Preferences:
Preferred Lodging: Preferred hotels: Fairfield Inn & Suites by Client, Fort Morgan Or Hampton Inn, Fort Morgan. Must ask for corporate rate when booking. No other lodging sites will be approved.
Air Travel: Acceptable, will put preference on candidates who are local.
Please detail any anticipated travel exceptions, for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission.

How many patients per day will there be? I see there are types of patients seen but needing clarification on the amount of patients per day? This is for hospital call only so it is hard to determine how many patients they may see. All depends on how many moms are due to deliver.

Active DEA - Required for credentialing (site will consider a transferred DEA, however, must keep this DEA for the duration of the assignment, not just for shifts worked.)
Must acknowledge candidate understands the EMR training requirements set forth in job description and will complete in this order to avoid missing their start date - Required
Candidate must be available for any of the stints in job description to be considered - Required
Clean MP/Background - Highly Preferred, candidates who present with a self inquiry NPDB report will be placed in priority over those that do not include this.
