Field Nurse Practitioner - Tucson, AZ
HealthCare Support is seeking a Field Nurse Practitioner to join a rapidly growing healthcare provider serving patients in Tucson, AZ .
This is a unique opportunity to deliver high-quality, patient-centered care directly in the home-focusing on Chronic Kidney Disease and primary care needs. If you're passionate about building relationships, improving outcomes, and working with autonomy while still being part of a collaborative team, this role is for you.

Schedule: Monday - Friday, 8:00 AM - 5:00 PM

Compensation Notes: $140,000 - $150,000 annually + 10% annual bonus
• Mileage reimbursement provided



Daily Responsibilities for the Field Nurse Practitioner:
• Conduct comprehensive in-home patient visits, including history and physical exams
• Develop and implement individualized treatment plans
• Order and interpret diagnostic tests and lab work
• Prescribe medications as appropriate
• Provide ongoing patient education and chronic disease management
• Complete accurate and timely documentation of services
• Collaborate with interdisciplinary care teams and participate in routine meetings
• Utilize telehealth to support patient care when appropriate

Required Qualifications for the Field Nurse Practitioner:
• Active Nurse Practitioner (NP) or Physician Assistant license in Arizona
• 2+ years of experience as a practicing NP or PA
