Case Manager

$22 - $25 per hour

Case Manager

Help residents build stability, life skills, and confidence as they transition into long-term housing. As a Case Manager, you will support in crisis support, referrals, and life-skills workshops in a supportive community setting.

What You'll Do:

  • Support special needs residents in overcoming housing barriers
  • Conduct intakes, assessments & goal planning
  • Facilitate onsite workshops & life-skills groups
  • Coordinate with community partners & property management
  • Maintain documentation & HMIS data
  • Create monthly resource newsletters & event calendars

What You Need:

  • Minimum 1+ year experience (homelessness, special needs, mental health, or related services)
  • High school diploma required (AA preferred)
  • Familiarity with Housing First & Harm Reduction
  • Strong communication & crisis-response skills
  • Valid CA driver's license + insured vehicle
  • Some evenings/weekends required

Pay Details: $22.00 to $25.00 per hour
