Case Manager

Case Manager
Help residents build stability, life skills, and confidence as they transition into long-term housing. As a Case Manager, you will support in crisis support, referrals, and life-skills workshops in a supportive community setting.
What You'll Do:
- Support special needs residents in overcoming housing barriers
- Conduct intakes, assessments & goal planning
- Facilitate onsite workshops & life-skills groups
- Coordinate with community partners & property management
- Maintain documentation & HMIS data
- Create monthly resource newsletters & event calendars
What You Need:
- Minimum 1+ year experience (homelessness, special needs, mental health, or related services)
- High school diploma required (AA preferred)
- Familiarity with Housing First & Harm Reduction
- Strong communication & crisis-response skills
- Valid CA driver's license + insured vehicle
- Some evenings/weekends required
Pay Details: $22.00 to $25.00 per hour
