Now Hiring: Case Manager

Responsibilities:

• Conduct comprehensive assessments of residents' needs, including medical, social, and housing needs.

• Develop individualized care plans in collaboration with residents, healthcare providers, and other stakeholders.

• Coordinate access to medical care, mental health services, substance abuse treatment, and other supportive services as needed.

• Advocate for residents' rights and access to resources, including housing, benefits, and community support services.

• Monitor residents' progress towards goals and adjust care plans as needed to promote positive outcomes.

• Provide crisis intervention and support to residents experiencing challenges or setbacks.

• Maintain accurate and up-to-date documentation of case management activities and resident progress.

Case Manager Requirements:

• Previous experience in case management or care coordination, preferably in a healthcare or social services setting

• Compassionate and empathetic attitude towards individuals from diverse backgrounds, including the underserved population.

• Knowledge of community resources and social service programs available to individuals in need

• Strong interpersonal and communication skills, with the ability to build rapport and establish trust with diverse populations

• Excellent organizational and time management abilities, with a keen attention to detail

• Ability to work independently and as part of a multidisciplinary team in a fast-paced environment

• Proficiency in electronic medical records (EMR) and documentation software preferred

• Valid driver's license and reliable transportation required
