The Lakes Home Care is seeking a licensed Nurse Practitioner (APRN) to support our home health operations and ensure compliance with Medicare guidelines. The NP will assist with Plan of Care documentation and conduct Face-to-Face patient encounters. Responsibilities include reviewing POC, performing required encounters, and collaborating with RNs, MDs, and staff to improve patient outcomes and documentation accuracy in a growing agency highly focused on compliance. #J-18808-Ljbffr The-Lakes-Home-Care
