Home Care Registered Nurse

Schedule

Sunday - Thursday 8am-4pm - on-call rotation and holidays required.

Assignment Details

Minimum Years of Experience: 2.0

Requirements

• Registered Nurse license required.
• Experience in home care and hospice preferred.
• Knowledge of OASIS documentation requirements.
• High-Tech skills including IV use and maintenance and complex wound care.

Responsibilities

• Assess patient and caregiver needs to provide quality care.
• Develop and communicate a realistic plan of care including patient and caregiver involvement.
• Serve as Case Manager for assigned patients, managing case loads and visit times.
• Implement a medical plan of care and nursing interventions as necessary.
• Evaluate care provided, addressing identified problems and adapting as necessary.
• Educate patients and caregivers on care plans, medications, and discharge instructions.
• Coordinate with interdisciplinary teams and ensure documentation is thorough and timely.
• Participate in peer review audits and case conferences to ensure quality of care.
