Responsibilities

About the Role:

CUCS seeks a Case Manager for The Hegeman, a 160-unit supportive housing residence in Brownsville, Brooklyn, serving low-income tenants, including those with histories of homelessness, mental illness, and HIV/AIDS. You will deliver person-centered case management to support clients in achieving housing stability, health, recovery, and wellness.

Key Responsibilities:

• Develop and update service plans; conduct ongoing assessments and monitor client progress.

• Connect clients to healthcare, benefits, housing, employment, and community resources.

• Provide crisis intervention, medication support, group facilitation, and daily living assistance.

• Deliver services onsite and in the field (home visits, escorting clients to appointments).

• Complete timely, accurate documentation and maintain compliance with funder standards.

Requirements:

• Education/Experience: HS Diploma + 2 yrs experience (CM I) up to Bachelor’s + 4 yrs experience (CM IV). College credits may substitute for experience.

• Skills: strong communication, organization, and crisis skills; MS Office proficiency; bilingual preferred.

• Physical/Field: Ability to travel across NYC, escort clients on transit, and lift up to 30 lbs. Valid driver's license may be required.

Benefits:
