Physical Therapist (PT) - Full-Time

Location: Menominee, MI
Facility: Menominee Health Services
Setting: Skilled Nursing Facility (SNF)

Continuum Therapy Partners is seeking a compassionate and dedicated Physical Therapist (PT) to join our rehabilitation team at Menominee Health Services in Menominee, Michigan.

This full-time Physical Therapist opportunity offers the chance to provide high-quality physical therapy services in a supportive, collaborative skilled nursing facility where your clinical expertise makes a meaningful impact every day.

Join an experienced rehabilitation team providing patient-centered care while working alongside Occupational Therapists, Speech-Language Pathologists, Physical Therapist Assistants, and other healthcare professionals. Treat primarily adult and geriatric patients while helping them improve mobility, strength, balance, safety, and functional independence.

• Evaluate patients and develop individualized physical therapy treatment plans.
• Provide skilled physical therapy interventions to improve mobility, strength, balance, endurance, and independence.
• Perform therapeutic exercises, gait training, mobility training, and functional activities.
• Monitor patient progress and adjust treatment plans based on patient needs.
• Collaborate with physicians, nursing staff, caregivers, and interdisciplinary rehabilitation team members.
• Complete accurate and timely documentation according to company, state, and federal guidelines.
• Educate patients and families on mobility strategies, exercise programs, and safety techniques.
• Provide compassionate, evidence-based physical therapy services.

• Graduate of an accredited Physical Therapy program.
• Current Michigan Physical Therapist (PT) license required.
• Skilled nursing or rehabilitation experience preferred but not required.
• New graduates are encouraged to apply.
• Strong communication, organization, and teamwork skills.
