As our Social Services Director, you will play a vital role in supporting residents\u2019 emotional, psychosocial, and discharge planning needs while serving as a compassionate advocate for residents and their families. Job Summary Responsible for the provision of Social Services to residents of the facility and their families under the direction of the Social Services Director. Must have working knowledge of interacting with residents of this facility. Responsibilities include meeting new residents upon admission, completing an Initial Psychosocial Assessment, visiting residents to follow up on concerns, and may include completing some MDS assessments under supervision. Arrange to purchase needed items for residents and ensure residents have adequate clothing items. Schedule visits to the dental doctor, eye doctor, podiatrist, and audiologist. Provide support in times of loss to residents and families. Assist residents in gathering documentation for support services such as Medicaid, Medicare, SSI, etc. Perform other duties as directed by the Social Services Director. Qualifications Minimum of a Bachelor\u2019s degree in social work; OR Minimum of a Bachelor\u2019s degree in a human-services related major (e.g., sociology, gerontology, psychology, rehabilitation counseling, or special education) AND One year in a healthcare setting where the individual works directly with individuals as a supervised social worker.
