We are seeking an experienced and detail-oriented Home Health Nurse to join our team and play a key role in delivering exceptional patient care. Many areas available!Looking for visit nurses! Wound Care experience a plus!Key Responsibilities:Develop individualized patient care plans based on assessment findings, physician orders, and patient goals.Educate patients and families on disease processes, medication management, safety, and self-care techniques.Coordinate multidisciplinary services, including therapy, nursing, and aide support.Ensure compliance with documentation standards, agency policies, and regulatory requirements.Communicate effectively with physicians, case managers, and other care team members.Provide compassionate, patient-centered care in the home setting.Qualifications:Current nursing license in the state of FL (required).Minimum 1–2 years of home health nursing experience with strong knowledge of OASIS documentation.Strong organizational, time management, and critical thinking skills.Excellent communication and interpersonal skills.Proficiency in EMR/home health documentation systems (we use Kinnser/WellSky, but Axxess, or similar experience preferred).Reliable transportation and valid driver’s license.What We Offer:Competitive pay with mileage reimbursement.Flexible scheduling options.Supportive team environment.Ongoing training and professional development opportunities.Health, dental, vision, and retirement benefits (for eligible employees). #J-18808-Ljbffr
