About the Role:

We are seeking a compassionate and skilled Registered Nurse (RN) to join our Per Diem team at Rock River Nursing & Rehab in Fort Atkinson, Wisconsin. As a Registered Nurse, you will be responsible for providing high-quality patient care, collaborating with interdisciplinary teams, and maintaining a safe and supportive environment for our residents. Responsibilities:

• Provide direct patient care to our residents, including assessments, interventions, and evaluations

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Monitor and report changes in patient condition to the healthcare team

• Maintain accurate and up-to-date patient records

• Participate in quality improvement initiatives to enhance patient outcomes

• Collaborate with other healthcare professionals to deliver comprehensive care

Requirements:

• As a Registered Nurse (RN) at Rock River Nursing & Rehab, you must:

  • Possess a current RN license in the state of Wisconsin

  • Have a minimum of 1+ years of experience in a long-term care or acute care setting

  • Demonstrate excellent communication, problem-solving, and critical thinking skills

Per Diem Position:
