Registered Nurse

Job Summary Reporting to the Care Manager, the Case Manager directs the care of clients under the home care program. Using strong assessment, organizational, and leadership skills, you identify client needs and work with the interdisciplinary team to facilitate appropriate internal and external resources. You coordinate care planning, facilitate client and family teaching, and develop care plans that ensure services are provided and modified on an ongoing basis. The role involves timely referrals to stakeholders and may require direct care, including wound care, palliative care, and home parenteral therapy. Job Description As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities, and populations while ensuring safety and well‑being. You will use nursing processes, critical thinking, problem‑solving, and decision making, and teach, counsel, and advocate on behalf of patients and families. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support – Health Care Provider (BCLS‑HCP) certification. If not practiced as a Registered Nurse within the past five (5) years, completion of the following free courses prior to employment: CRNA Medication Management (Chapters 1‑4) and NextGenU Health Assessment Resources Certification. Additional Required Qualifications Physically able to perform all assigned tasks, including climbing stairs, walking outdoors, lifting, pulling, pushing, stooping, and carrying, which may involve visits to client homes that are not ideal. Valid Driver’s license and access to a reliable vehicle. Preferred Qualifications Minimum of 2 years Home Care experience within the last 5 years of nursing; relevant experience in community nursing also considered. Current use of Connect Care, Service Authorizations, RAI‑HC, Netcare, and AADLs. Experience with HPT, wound care, client teaching, and palliative support. Excellent interpersonal, communication, time‑management skills, and ability to work collaboratively in a flexible, adaptable team environment.
